Admissions
When registering for admissions, please have the following information available:Insurance card
Photo identification
Contact information
Emergency contact information
Social Security number
Current list of medications, medical history, food and medication allergies
Provider's orders
Since RMCHCS does not assume responsibility for a patient's valuables, we recommend that valuables be sent home with a family member. However, in the event that this is not possible, RMCHCS' security department will assist the patient in storing valuables during his/her hospital stay.
Discharge
Once a provider has ordered a patient's discharge, a nurse will provide written instructions for care at home. The nurse will review these documents with the patient and family before they leave the hospital. It is important that the patient and/or the family make sure they understand all the instructions before going home. Please follow up with health care team caring for you about any health or discharge concerns prior to discharge. Do not hesitate to ask for clarification of instructions, if needed.The RMCH Case Management team is available throughout your hospital stay to discuss your individual health care options for transitioning out of the hospital when you are medically ready for discharge.
